- Visit Your VA Provider: Ask your VA primary care provider or care coordinator for a referral to: Crystal Clear Dental Spa — TMJ, Esthetics & Sleep Medicine, Jacksonville, FL (Optum / CCN Region 3).
- VA Issues a Referral (SEOC): The VA determines eligibility and creates a Standardized Episode of Care (SEOC) for TMJ or sleep-related breathing disorders. Without this referral, the VA cannot authorize outside-the-VA treatment.
- We Provide Your Specialized Care: Once the referral is approved, we can evaluate and treat TMJ dysfunction, chronic jaw or facial pain, oral appliance therapy for sleep apnea, bite and orthotic therapy, and headache and muscle imbalance patterns.
- Follow-Up & Coordination With VA: We communicate directly with your VA care team to ensure coordinated and compliant treatment.
